UCR USDOT Numbers and Indiana Operating Authority Checklist

Below you will find a checklist of the items needed for your Unified Carrier Registration, USDOT Numbers, and/or Indiana Operating Authority.

Unified Carrier Registration (UCR)

New interstate carriers (crossing the state lines), brokers, leasing companies, and freight forwarders:

Existing interstate carriers (crossing the state lines), brokers, leasing companies, and freight forwarders seeking renewal:

USDOT Numbers

New Intrastate (never leave Indiana) for-hire carriers (hauling someone else's property) or private property carriers of hazardous materials:

Renewal requirements for USDOT numbers:

  • Form IOA-1 (or preprinted form IOA-1A)
  • Proof of insurance

New Intrastate (never leave Indiana) private carriers (hauling your own private property) hauling nonhazardous materials:

Please note that there is not a renewal process if you are an intrastate private carrier hauling nonhazardous materials.

Indiana ID Numbers

Intrastate (never leave Indiana) for-hire carriers (hauling someone else’s property) with a gross vehicle weight rating or combined gross vehicle weight rating that is less than 10,000 pounds:

Indiana Operating Authority

Those that transport passengers or household goods for hire must apply for Indiana Operating Authority. Because there are several types of operating authorities (permanent, temporary, and emergency), it is important to review the options, requirements, and applications.

All incomplete applications for passenger and household goods will be returned. If there are any issues once we receive the complete application, we will return the application for corrections. Once corrections have been made, return the entire completed and corrected application. You must schedule an appointment if you would like to visit our walk-in customer service center to set up your account.
